Documentation update + code tweaks

* write-makefile: bind the *default-pathname-defaults*, so that filenames
 specified in *lisp-setup-dependencies* should be relative to the project
 being migrated. Simplify and rework example.
* Makefile: add an intermediate target install.
* Documentation: mainly update the TODO.
