Do a FINISH-OUTPUT at the end of a transaction commit (suggested by Marco Baringer).

Add :KEY-KEY and :VALUE-KEY initargs to btrees.

Add some standard slot indexes.

Add :UNIQUE initarg for persistent slots (not finished yet).
