Use hash table for libffi-type-pointer

Use hash table for libffi-type-pointer instead of a class slot, and
simplify #'libffi-type-pointer methods.  Remove obsolete package
definition file.  Tests in cffi-tests not run due to error in load,
but GSLL tests pass.
